1. I/We, the parents/guardian of the named candidate for a position on the Eastpointe Youth Baseball
League, hereby give my/our approval to participate in any and all activities.
2. I/We know that participation in baseball may result in serious injuries and protective equipment does not
prevent all injuries to players, and do hereby waive, release, absolve, indemnify and agree to hold harmless the Eastpointe Youth Baseball League, the organizers, sponsors, volunteers and participants from any claims arising out of any injury to my/our child whether the result of negligence or for any other cause.
3. I/We certify that the birthdate listed is correct and will be willing to furnish a birth certificate of the named candidate upon request.
4. I/We grant permission to Eastpointe Youth Baseball League the right to use, reproduce and/or distribute
photographs, films, videotapes and sound recordings of my/our child without compensation or approval rights, for use in materials created for the purposes of promoting the activities of EYBL including in the media and on the internet.
5. I/We understand that any loan equipment must be returned at the end of the year.
